Sunrise
Health Service Coordinated Care Trial
Sunrise is one of five second round Coordinated Care Trials, which
are part of the Commonwealth Government's Enhanced Primary Care Package.
The goal of these trials is to provide additional benefits to clients
and communities through the coordination and integration of care and
effective use of resources. CRH is the Local Evaluator for the new
trial and the Katherine staff are working closely with the National
Evaluator, PricewaterhouseCoopers, and the Sunrise staff in the detailed
evaluation of the trial, which will continue until mid 2005. |

Katherine
West Health Board Chronic Disease Self Management Program
This is one of the twelve national demonstration projects of the Department
of Health and Ageing's Sharing Health Care Initiative,
which is also part of the Enhanced Primary Care Package. The
objectives of the Sharing Health Care Initiative are to improve
health-related quality of life for people with chronic diseases; to
improve the use of the health care system by people with chronic conditions;
and for individuals, families and health care professionals to work
together in the management of chronic conditions. CRH Katherine leads
the local evaluation as part of the Cooperative Research Centre in
Aboriginal Health. |

Katherine
Regional Aboriginal Health and Related Services
(KRAHRS)
This alliance between the four Aboriginal community controlled organisations
in Katherine continues to develop and has potential
to work further with CRH in investigating innovations in health
service provision to remote Aboriginal communities. KRAHRS
manages the Outreach Allied Health Project, which is funded as
part of the Department of Health and Ageing's Regional Health
Services program. CRH Katherine is conducting the evaluation of
this project. |

Remote Home
and Community Care (HACC) Review
CRH Katherine is participating in the review of the NT HACC
program for the Top End. The Commonwealth and Northern Territory
Governments are looking to develop a clear process and guidelines
to measure the extent to which remote HACC providers meet national
standards. The outcome of the project will be a revised and simplified
assessment instrument which is more appropriate and applicable to
remote Indigenous communities.
